5th gear pop-out refresher.

It's finally happend to me. On a long road trip the problem came about, and
slowly got so bad that I dare not even try to put the car in 5th. I know
this subject has been covered countless times before, but I've forgotten
some of the details.
What is the gneral concensious?
Is this caused by bad engine mounts?
Once the problem has accured, is it too late to do anything
short of replacing transmission internals?
What can be done to fix the problem temperarily / for ever?
